Welcome to SPS Health Information where our on site staff Nurses can keep parents informed on seasonal health issues and alerts, if necessary.

Immunizations
Newly enrolled students must submit an immunization record at our enrollment center.

Required immunizations for Pre-K enrollment include: 
DTaP (4)
Polio - IPV (3)
MMR (1)
Hep B (3)
Hep A (2)
Varicella (1)

Required immunizations for new kindergarten enrollment include:
DTaP (5)

MMR (2)
Hep B (3)
Hep A (2)
Varicella (1)

7th grade students are required to have TDAP before the school year begins and must submit the record at Sapulpa Middle School in order to receive their schedule.

  • List of vaccines required to attend school in Oklahoma. 
  • For school enrollment, a parent or guardian shall provide one of the following: Current, up-to-date immunization records or a completed and signed exemption form.
  • Creek County Health Department provides some immunizations for children free of charge

    Exemption
    All students of all grade levels have a required & current immunization record or a state approved exemption form on file with the school prior to their admission to school AND this information is made available to parents on the school website. (OK state law 70 O.S. ยง 1210.191).

    Documentation: Assurance Letter signed by Principal verifying a current record of immunization or exemption form (Spanish version) is on file for each student & is made available for review and a website printout or screen shot showing parent information.
